Butte County medical group seeks Case Manager LVN
Full-Time 40 Hours per week!
Bring your medical and psycho-social case management skills and join the team!

Whats it all about:
Case management and care coordination.

Duties, responsibilities and required skills:
Facilitate referrals, linkages and coordination of care
Authorization for supporting medications
Work with behavioral health clinicians
Referrals for crisis and counseling
Partner with license staff for surgical letters and legal name changes and notifications
Advocate for transgender and gender expansive patients
Create referral and resource list of supporting agencies, services and providers

Required:
BLS
LVN License
Experience with LGBTQ community
Preferred: Case management experience
Experience with prior authorizations for medical management
Bachelors Degree in Social Work or 2 years experience working with the transgender and gender expansive community

Schedule: Monday thru Friday 8:00am to 5pm
