Job Description

Butte County medical group seeks Case Manager LVN
Full-Time 40 Hours per week!
Bring your medical and psycho-social case management skills and join the team!

Whats it all about:
Case management and care coordination.

Duties, responsibilities and required skills:

  • Facilitate referrals, linkages and coordination of care
  • Authorization for supporting medications
  • Work with behavioral health clinicians
  • Referrals for crisis and counseling
  • Partner with license staff for surgical letters and legal name changes and notifications
  • Advocate for transgender and gender expansive patients
  • Create referral and resource list of supporting agencies, services and providers

    Required:
    BLS
    LVN License
    Experience with LGBTQ community
    Preferred: Case management experience
    Experience with prior authorizations for medical management
    Bachelors Degree in Social Work or 2 years experience working with the transgender and gender expansive community

    Schedule: Monday thru Friday 8:00am to 5pm
